ВЛИЯНИЕ ТИРЕОИДНОЙ ДИСФУНКЦИИ НА ПАТОЛОГИЧЕСКУЮ ГЕСТАЦИОННУЮ ПРИБАВКУ ВЕСА

Аннотация

Имеющиеся на сегодняшний день данные свидетельствуют о наличии связи между продукцией тиреоидных гормонов в период беременности и гестационным увеличением массы тела. Более высокий уровень тиреотропного гормона (ТТГ) и гипотиреоз у матери связаны как с более высоким индексом массы тела (ИМТ) до беременности, так и с увеличением риска чрезмерной гестационной прибавки веса. При повышении уровня свободного тироксина (T4св) и гипертиреозе у беременных женщин наблюдается противоположный эффект. Уровень трийодтиронина (Т3), в свою очередь, значительно выше у беременных женщин с избыточной массой тела и с ожирением. Что касается перинатальных исходов, по результатам многих исследований было выявлено, что изменение материнских концентраций ТТГ, T4св и T3 влияет на массу тела плода, а гестационная прибавка веса матери способна модифицировать данный эффект. Однако имеются научные публикации, свидетельствующие, что несмотря на связь гипо- и гипертиреоза с гестационным приростом веса, дисфункция щитовидной железы матери в период беременности не оказывает влияния на массу тела новорожденного. Итак, понимание роли эндокринных факторов в патогенезе нарушений липидного обмена, в том числе патологической прибавки веса при беременности, создает реальную возможность повлиять на массу тела женщины и ассоциированные с ее девиациями акушерские и перинатальные исходы.
